The Frontenac Lennox and Addington Ontario Health Team (FLA OHT) is making significant strides in connecting local residents to essential health services.
Ontario Health Teams were created under the Ministry’s Connected Care for All Act to redesign health systems by listening to community needs and building a system that serves people effectively.
myFM spoke with Executive Lead Dr. Kim Morrison, to learn more about the FLA OHT, which launched the fall of 2020.

The foundation of the OHT’s approach is primary care, with the goal of attaching everyone in the area to a team-based care model.

In addition to primary care, FLA OHT offers Integrated Care Pathways for patients with chronic diseases like heart failure and lung conditions. These pathways provide patients with personalized care plans and connect them with specialists and community resources. Palliative care services are also streamlined through the FLA OHT, ensuring seamless support.
For those without a doctor, the FLA OHT encourages registration with Health Care Connect, and access clinics are available for immediate needs.
